SummaryThe CNA provides direct, quality care to resident in a timely, accurate and consistent manner to assist the resident in achieving maximum independence, social interaction, sense of identity and choice. Provides the highest quality direct skilled care in a timely, efficient, accurate, safe and consistent professional manner to assist residents in achieving maximum independence, social interaction, sense of identity and choice. Provides Quality Service in accordance with policies and procedures with dignity, respect and privacy. Maintain public relations through professional and positive attitude and behavior while providing quality service to all customers. Maintains a safe secure and clean environment for all customers. Actively participates in quality initiatives that improve the healthcare status of those we serve.

Experience/Education Requirements

  • High school diploma or equivalent preferred

License Or Certification Requirements

  • Nursing assistant certification
  • Successful completion of NACEP within 120 days of start
  • Current CPR certification required

Responsibilities

  • Actively participates in nursing department meetings, town meetings, employee opinion survey, provide communication and feedback with development, implementation and evaluation of delivery of care, and RMC health system initiatives.
  • Actively participates in self-development by attending educational programs and has completed 12 annual hours of continuing education annually.
  • Serves as mentor to newly hired staff completing evaluations, attending meetings, and demonstrating care.
  • Applies alarms and restraints properly and assures that they are functioning properly.
  • Serves as clinical partner to the nurse giving input into challenging care issues. Reports and/or documents any changes observed in condition or behavior of resident throughout shift. Communicates creative ideas and methods to other team members for continuity. Collaborates with Nurse Team Leader in developing and supporting work assignments. Reports all unusual conditions, occurrences, complaints or reactions to the nurse who has responsibility for that resident in a timely manner.
  • Delivers thorough report of resident's status to oncoming shift at shift change in a private area. Discuss personal issues with resident, family, coworkers in a private area and professional manner. Updates POC and communicates changes to coworkers.
  • Spend free time with residents providing conversation and companionship.
  • Help residents with turning/repositioning, bathing, grooming, dressing, hygiene, toileting, supervision or providing feeding assistance, splinting/bracing, ambulation, ROM and activity and therapy program attendance. Provide resident specific care routines to ensure residents' comfort and safety. Transfers residents in a safe manner using proper body mechanics, equipment and resources available to determine how much assistance resident requires.
  • Has participated in evacuation/fire drills.
  • Maintains a clean and safe work environment for residents and staff. Responsible for equipment assigned for resident care is clean. Reports and removes any equipment that is not functioning properly. Acts to prevent accidents and incidents.
  • Promptly answers call lights in a timely manner. Anticipates resident needs, checks on resident regularly and demonstrates cooperative caring attitude when assisting residents. Keeps call lights within reach of resident before leaving room. Provides orientation/reorientation of residents to their environment and provides redirection as appropriate for resident’s level of understanding. Provides each resident some privacy by knocking on doors and greeting residents prior to entering room, pulling curtain and/or closing door while rendering personal care. Discuss personal issues with resident, family, co-workers in a private space/area in a professional manner. Assures hygiene and mobility needs are met every 2 hours and PRN while the resident is in restraint.
  • Provides tray service and feeding of residents. Assist with preparation, delivery and documentation of meals, nourishments and snacks and fluids as appropriate.
  • Speaks to residents in a pleasant helpful (resident center) manner, avoiding talking in halls or common areas with other staff related to resident care needs or other issues such as staffing, assignments or concerns.Models professionalism in communication using sensitivity, active listening techniques, and diplomacy with all customers.
  • Understands and implements the principals of infection control, consistently using standard precautions and washes hands after each resident contact. Removes and disposes of gloves after each resident contact.
